Athlete Nomination

• Graduated from Cedar Creek School a minimum of 10 years prior to nomination
• Participated in athletics at Cedar Creek High School
• While athletic achievement is of prime importance, moral character, positive role model and good citizenship during high school and post high school years will be taken into consideration.

Coaching Nomination

• Made a significant contribution to Cedar Creek athletics and have demonstrated good character and citizenship.
